Medical Information and Waiver Forms

The Medical Information and Waiver Form for any tournament must be completed by the athlete's parent/guardian.  That may be accomplished in one of two ways:

1) Each parent / guardian may access this page online and complete the form for his/her child.

2) For families that do not have internet access...You may distribute this paper form to such families.  Those families should complete the form and return it to the school.  Then someone from the school (coach, athletic director, etc) should enter the form online (via the online forms below), on the parent/guardian's behalf.  In this case, you must bring the original paper form and give it to NCSAA during on-site registration.
